Chronic Disease Management
Chronic disease management is what General Practitioners do best. Your GP is highly skilled in this area and the coordination of your care with other health practitioners. It is important to make regular appointments to keep track of your chronic disease – even when you feel well.
Your GP is best placed to inform you if you are eligible for a Chronic Disease Management Plan. This working document helps you and your GP set goals, monitor progress, and keep track of other providers you may need to see.
Chronic illnesses include Diabetes, Heart Disease, Chronic lung disorders (asthma, COPD), Kidney disease, cancers, and many more.

Telehealth appointments
Telehealth is still available for current patients. It may be appropriate if you have a respiratory illness or are caring for someone. We do encourage face-to-face appointments where possible as this allows a more thorough medical assessment. The cost of a telehealth appointment is the same as the face to face appointment.
-
Focussed respiratory clinics
The Doctors at PCMP have continued to provide face-to-face appointments to people with cold and flu symptoms to allow timely physical assessment. They do this by offering appointments in our “respiratory clinic” which runs daily for 3 hrs. Patients book these appointments by phone (no online booking for this clinic). Upon arrival patients wait in their car until the Doctor calls them into the back room of the clinic for the consultation. Payment is made over the phone as this ensures no one with respiratory symptoms will be in the waiting room, thus decreasing risk of viral spread to other, possibly vulnerable, patients.

For your first appointment, we recommend you schedule a longer appointment.

We apologise in advance for emergency delays that prolong your waiting time. Please inform the receptionist upon arrival to your appointment if you require urgent attention.

Results
Results for tests ordered by the doctors at our practice are best discussed at a follow-up consultation which can be made by reception at the end of your appointment. It is important to discuss the most appropriate method of getting these results with the doctor at the time of the consultation.
Prescriptions
In order to ensure an uninterrupted supply of your medications it is important to check with your GP at each consultation to ensure you have enough prescriptions to last until the next appointment. Pharmacies usually advise you if you have just filled the last repeat and that is an excellent opportunity to contact the practice for an appointment so your doctor can review the ongoing prescription of that medication and make sure it’s still right for you.
If, for unforeseen circumstances, you run out of a particular prescription, you should contact the practice and the reception staff may be able to offer you a “5 min telephone script consultation” with your regular GP. There is a small fee associated with this.
